    Download CRA consent form RC161 and print a copy to sign.

  2. Complete and sign

    Fill in your details, then sign the form with a handwritten signature.

  3. Upload below

    Upload the signed copy together with your contact information using the form.

CRA will not discuss your file with our office without a handwritten signature.
